Initiates preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety. Develops, implements and updates the nursing care plan and provides skilled nursing care as outlined in the nursing care plan. Conducts an initial evaluation and completes the hospice and/or home health admission process for all appropriate patients. Participates in updating nursing care plans and medication sheets. Observes, records and reports to supervisor on the general physical and mental condition of the patient.

Participates in in-service education programs, a minimum of 4 hours per year, patient care conferences and staff meetings. $13,000 Student Loan Repayment or $25,000 Sign-on Bonus for individuals who have not previously participated in this program.
